Bone health is one of the most clinically significant health concerns that emerges in the years following menopause. The relationship between estrogen and bone density is direct and well-established. Estrogen acts as a brake on the process of bone resorption, the natural cycle in which old bone tissue is broken down before new bone is formed. When estrogen levels fall, this brake is released and bone loss accelerates, sometimes dramatically, in the first five to seven years after the final period.

How Bone Loss Happens

Bone is living tissue in a constant state of remodeling. Specialized cells called osteoclasts break down old bone while osteoblasts build new bone. In younger years, this process is roughly in balance or favors bone formation. After menopause, osteoclast activity increases relative to osteoblast activity, and the result is a net loss of bone density over time. The trabecular bone found in the spine, hips, and wrists, which is more metabolically active than cortical bone, is particularly vulnerable.

Risk Assessment

Not all women face the same level of risk. Factors that increase the rate of bone loss after menopause include early menopause, low body weight, smoking, high alcohol consumption, a family history of osteoporosis, certain medications including corticosteroids and proton pump inhibitors, and low dietary calcium and vitamin D intake throughout life. A bone density scan, called a DEXA scan, is typically recommended for women at 65 or earlier for those with significant risk factors.

Nutrition for Bone Preservation

Adequate calcium intake remains important throughout the postmenopause years. The recommended daily intake for women over 50 is 1200 milligrams from food sources, with supplementation filling any gap. Dairy products, fortified plant milks, sardines with bones, and dark leafy greens are reliable food sources. Calcium carbonate supplements are absorbed best with food, while calcium citrate can be taken any time.

Vitamin D is equally important and often deficient. It is required for calcium absorption and bone mineralization and also supports immune function, muscle strength, and mood. The current recommended intake for adults over 70 is 800 IU daily, though many practitioners recommend higher levels based on blood testing. Brief daily sun exposure supports vitamin D synthesis in the skin, though supplementation is often needed, particularly in northern climates during winter months.

Exercise for Bone Strength

Weight-bearing exercise is one of the most powerful tools available for preserving bone density. Walking, hiking, dancing, and resistance training all stimulate bone formation by placing mechanical load on the skeleton. Women who combine weight-bearing cardio with progressive strength training show significantly better bone density outcomes than those who exercise minimally. Regular monitoring with repeat DEXA scans every two to five years allows clinicians to track whether interventions are working and adjust as needed.
